A correctly fitted martingale should consist of 2 loops, 1 large adjustable loop which goes over the head and sits around the neck and a smaller loop with a d-ring at the front. When a lead is attched to the d-ring for walking the loop will pull closed and tighten the collar so that it is unable to slip off over the dogs head. 
It is NOT recommended to leave a martingale collar on a dog unattended due to the tightening nature of the collar a dog may become stuck and unable to escape.
